Physical Therapist (PT) - Home Health Case Manager - South City Chicago

Join us at Advocate Health as a Physical Therapist (PT) - Home Health Case Manager in South City Chicago. This role offers an opportunity to work independently within a dynamic, integrated healthcare organization, providing home-based therapy services.

Position Details
  • Location: Southeast City, focusing on ZIP codes 60620, 60619, 60617, 60643, 60628
  • Hours: Day shifts
  • Sign-on Bonus: Up to $10,000 (subject to taxes; internal candidates are not eligible)
Responsibilities
  • Assess and address mobility, safety, communication, and equipment needs of patients in their homes
  • Establish strong relationships with patients and families
  • Consult with physicians and healthcare providers as needed
  • Travel extensively within the designated geographical area, including in all weather conditions
  • Work rotating weekends and holidays, adhering to scheduled hours
  • Perform physical tasks including bending, sitting, standing, climbing stairs, lifting (up to 35 pounds), and assisting with patient transfers
  • Develop and evaluate personalized physical therapy treatment plans to promote patient self-care
Qualifications
  • Licensed Physical Therapist in Illinois or eligible for licensure
  • At least 1 year of clinical experience or equivalent affiliation experience
  • Proficient with home therapy equipment and assistive devices
  • Basic computer skills and ability to learn documentation systems
  • Strong communication, documentation, interpersonal, and organizational skills
