Physical Therapist (PT) - Home Health Case Manager - South East Chicago

Join us as a Physical Therapist (PT) - Home Health Case Manager at Advocate Health, located on the Chicago Southeast Side. This full-time position offers a sign-on bonus of up to $10,000, with eligibility conditions.

Key Responsibilities
  1. Quality of Care: Ensure delivery of quality, cost-effective care, utilizing evidence-based practices, participating in quality improvement initiatives, and promoting patient safety.
  2. Care Coordination: Collaborate within the healthcare team, participate in meetings, and support organizational initiatives to optimize patient outcomes.
  3. Leadership & Professional Development: Make informed clinical decisions, adapt to changing situations, and communicate effectively using techniques like SBAR and 5P's.
  4. Patient Relationships: Build restorative relationships, educate patients and families, and involve them in care planning.
  5. Business Development: Adhere to policies, optimize resource utilization, and contribute to organizational growth.
  6. Patient Education: Assess learning needs, provide tailored education, and promote self-care and health literacy.
Qualifications
  • Master's Degree in Physical Therapy and licensure in Illinois (or eligible)
  • Valid driver’s license
  • Basic Life Support (BLS) certification within 6 months of hire
Skills & Abilities
  • Computer literacy, strong documentation, interpersonal, and organizational skills
  • Proficiency with home therapy equipment and assistive devices
Physical Requirements & Conditions
  • Travel within the service area, working on weekends and holidays as scheduled
  • Ability to lift up to 35 pounds, assist with patient transfers, and work in diverse home environments
