Job summary

Elara Caring is seeking a Physical Therapist to join our home health team in Texas. You will evaluate patients, develop and implement individualized treatment plans, and provide high-quality PT services in patients’ homes.

Responsibilities include monitoring progress, coordinating with physicians and caregivers, and maintaining detailed documentation. APTA-approved program graduate with an active PT license and at least one year of healthcare experience is required.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
