Job summary

Elara Caring is seeking a licensed Physical Therapist to join our home care team serving the West Houston market, including Houston Heights, Jersey Village, Katy, Sugar Land, Missouri City, Richmond, Rosenberg, and surrounding communities. You will evaluate patients, develop individualized treatment plans, and provide PT services in patients' homes.

Qualified candidates have an APTA-approved program, an active PT license for Texas, and at least one year of healthcare facility experience.

• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.

• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.

This position supports patients throughout the West Houston market, including areas such as Houston Heights and Jersey Village; Katy; Sugar Land and Missouri City; Richmond and Rosenberg; and surrounding communities. Specific patient assignments will be determined based on clinician location and business needs.

As a Physical Therapist, you'll contribute to our success in the following ways:

• Evaluate patients and develop individualized treatment plans.

• Provide physical therapy services in the home setting.

• Monitor patient progress and adjust care plans as needed.

• Collaborate with physicians, caregivers, and the interdisciplinary care team.

• Complete timely and accurate clinical documentation.

• Supervise therapy assistants and support staff, as appropriate.

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program

• Current, unrestricted PT license valid for the state of work

• Minimum one (1) year of experience in a healthcare facility

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
