Elara Caring is seeking a dedicated Home Health Physical Therapist to cover Wood and Hancock County. You’ll provide direct patient care in home settings, evaluate functional needs, and create individualized care plans.

Travel up to 50% is required, with opportunities for professional growth and a competitive benefits package. Qualified candidates have an APTA-approved PT program, an unrestricted PT license, and at least one year of healthcare facility experience.

Qualifications

• Graduate of an APTA-approved physical therapy program.

Responsibilities

• Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists to adjust care plans for optimal outcomes.
• Design and implement a plan of care for patients based on thorough assessments and involve significant others where appropriate.
• Document evaluations, treatment goals, and plans; maintain clear and timely records and signatures.
• Lead and collaborate with staff to promote physical therapy services and community health initiatives.

Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes.* Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.* Conducts therapy practice within defined standards of care.* Directs the assistance of support staff to achieve patients’ functional goals.* Documents evaluations, treatment goals, and plans.
