Job summary

Elara Caring in Houston, TX is seeking a dedicated Physical Therapist to deliver high-quality home-based care across the West Houston market, including Houston Heights, Jersey Village, Katy, Sugar Land, Missouri City, Richmond and Rosenberg. You will evaluate patients, create individualized treatment plans, and provide therapy in patients' homes, coordinating with physicians and the care team.

Responsibilities include monitoring progress, updating plans, documenting outcomes, and supervising

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

This position supports patients throughout the West Houston market, including areas such as Houston Heights and Jersey Village; Katy; Sugar Land and Missouri City; Richmond and Rosenberg; and surrounding communities. Specific patient assignments will be determined based on clinician location and business needs.

As a Physical Therapist , you’ll contribute to our success in the following ways:
