Job summary

Elara Caring is seeking a dedicated Physical Therapist to deliver high-quality, home-based therapy across assigned patient homes in Texas. You will evaluate patients, tailor treatment plans, and provide hands-on PT services in the home setting, ensuring safety, efficacy, and compassionate care.

Collaborate with physicians and the care team, monitor progress, and document outcomes meticulously. The role also includes supervising therapy assistants and continuing education opportunities for staff.

Qualifications

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver’s license or reliable transportation for travel between patient homes.

As a Physical Therapist , you’ll contribute to our success in the following ways:

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
• Current, unrestricted PT license valid for the state of work
• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
