Job summary

Elara Caring is seeking a Physical Therapist to provide home-based care, evaluate patients, and develop individualized treatment plans. You will work with diverse care teams to ensure the right care at the right time, in the right place.

The role requires a current PT license and at least 1 year in a healthcare facility, plus a valid driver's license or reliable transportation to reach patient homes. Comprehensive benefits accompany this position.

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
• Current, unrestricted PT license valid for the state of work
• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
