Job summary

Elara Caring is seeking a dedicated Physical Therapist to join our home-based care team in Connecticut. The role focuses on delivering high-quality, individualized PT services in patients' homes, ensuring safety and effective progress.

A current PT license and driving to patient sites are required. You will evaluate patients, develop plans, and collaborate with physicians and caregivers, while maintaining thorough documentation and supervising staff as appropriate.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Ability to travel between patient homes and assigned work locations.
• Ability to lift, push, pull, and move up to 50 pounds.

What is Required?

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
