Job summary

Elara Caring is seeking a dedicated Physical Therapist to join our in-home care team. You will evaluate patients, develop individualized treatment plans, and deliver therapy in patients' homes, coordinating with physicians and caregivers to ensure optimal outcomes.

The role requires an APTA-approved degree, current PT licensure, and at least one year of healthcare facility experience, with reliable transportation for travel between homes. You’ll also document care and may supervise staff.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license or reliable transportation to travel between patient homes.
• Ability to frequently lift, push, pull, and move up to 50 pounds.

Responsibilities

What is Required?

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
