Job summary

Elara Caring in Argos, IN is seeking a dedicated Physical Therapist to join our home-based care team. The PT will evaluate patients, develop individualized treatment plans, provide therapy in patients' homes, and monitor progress while coordinating with physicians and care teams.

The role requires an APTA-approved PT program graduate, an active PT license in Indiana, at least 1 year of healthcare facility experience, and a valid driver’s license with reliable transportation to travel between

What is Required?

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
• Current, unrestricted PT license valid for the state of work
• Minimum one (1) year of experience in a healthcare facility
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
