Job summary

Elara Caring is seeking a Physical Therapist to join our home-based care team. You will evaluate patients, develop individualized treatment plans, and provide PT services in the home setting, delivering Right Care at the Right Time.

You will collaborate with physicians and care team, monitor progress, document accurately, and supervise therapy assistants as appropriate. A valid PT license and driving requirement are essential.

Qualifications

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one year of experience in a healthcare facility.
• Must possess a valid driver's license or have reliable transportation to travel between patient homes.
• Ability to frequently lift, push, pull, and support up to 50 pounds.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
