Job summary

Interim HealthCare of Northern Illinois is seeking a Home Health Physical Therapist to restore mobility and function in patients' homes. You will assess, treat, and tailor plans of care to individual daily life while collaborating with an interdisciplinary team.

Requirements include Illinois PT license, graduation from an accredited program, CPR, and at least 1 year of PT experience in home health. We offer competitive pay, mileage, and growth opportunities.

Qualifications

• Graduate of an accredited Physical Therapist Program and Illinois PT license.
• Minimum of 1 year PT experience, ideally in home health.
• Current CPR certification.

Responsibilities

• Provide home-based physical therapy to regain mobility and function.
• Work with a home health team to plan and adjust care.
• Observe movements, document progress, modify care plan as needed.
• Assist patients with exercises and hands-on therapy to manage pain.
• Educate patients and families on exercises and safety at home.
• Support patient goals and promote independence.
