Job summary

Interim HealthCare is seeking a dedicated Home Health Physical Therapist to deliver personalized care in patients' homes across Rockford, IL and surrounding areas. You’ll work with a multidisciplinary team to implement individualized plans of care for mobility-impaired patients.

Qualifications

• Graduate of an accredited Physical Therapist program and active PT license in Illinois.
• Minimum of 1 year of physical therapy experience, ideally in home healthcare.
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Good clinical judgement and compassionate interpersonal skills.

Responsibilities

• Provide home-based physical therapy to patients with mobility limitations in their homes.
• Work as part of a home health team to implement the plan of care and goals.
• Observe movements, assess progress, and modify care plans as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients and families about the plan of care, exercises, and safety.
