Job summary

Interim HealthCare of NE Florida is seeking a Home Health Physical Therapist to support patients in Jacksonville, FL and surrounding areas. You’ll provide one-on-one, in-home therapy focused on restoring strength, mobility and function.

Qualifications

• Graduate of an accredited Physical Therapist program and FL PT license.
• Minimum of 1 year PT experience, preferably in home health.
• CPR certification.
• Knowledge of home health regulations.
• Strong interpersonal skills and compassion.

Responsibilities

• Provide home-based physical therapy to patients in their homes.
• Collaborate with home health team (RN, LPN, CNA, HHA, OT, SLP) to achieve care goals.
• Assess movements, document progress, and adjust care plan.
• Guide patients and families through exercises and self-care.
• Educate on plan of care and safety during activities.
