Interim HealthCare in Rockford, IL is seeking a Home Health Physical Therapist to help patients regain strength and mobility in their homes.

You will work with a home health team, provide therapy, assess progress, educate patients and families, and follow the plan of care. The role offers a sign-on bonus, mileage reimbursement, 1:1 patient ratios, flexible assignments, EMR, training and CEUs, plus tuition discounts and comprehensive benefits.

Qualificações

• Graduate of an accredited Physical Therapist Program and active PT license in Illinois.
• Minimum of 1 year of physical therapy experience, ideally in home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Good clinical judgement, strong interpersonal skills, dexterity and compassion.

Responsabilidades

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to illness or injury.
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals.
• Observe movements, conduct assessments, document progress and modify care plan as needed.
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility.
• Educate patients on their plan of care, established goals, exercises and self-care.
• Coach family on the plan of care, therapy goals and safely assisting with exercises.
