Job summary

Interim HealthCare in Janesville, WI is seeking a Home Health Physical Therapist (PT) to deliver patient-centered therapy in a home setting. You will help patients regain strength, manage pain and achieve functional goals, often working with an interdisciplinary team.

This full-time role requires a Wisconsin PT license, graduation from an accredited program and at least one year of experience. We offer flexible assignments, competitive pay, mileage reimbursement and opportunities to earn CEUs.

Qualifications

• Graduate of an accredited Physical Therapist Program and active PT license in Wisconsin.
• Minimum of 1 year of physical therapy experience, ideally in home healthcare.
• CPR certification.

Responsibilities

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury.
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals.
• Observe movements, conduct assessments, document progress and modify care plan as needed.
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility.
• Educate patients on their plan of care, established goals, exercises and self-care.
• Coach family on the plan of care, therapy goals and safely assisting with exercises.

Requirements
