Interim HealthCare of Northern Illinois is seeking a Home Health Physical Therapist (PT) in McHenry, IL. You will provide individualized therapy in patients' homes to improve strength, mobility and pain management while coordinating with a multidisciplinary team.

Requirements include an Illinois PT license, graduation from an accredited PT program, CPR certification, and at least one year of home health or equivalent experience. Competitive salary and benefits offered.

Qualifications

• Graduate of an accredited Physical Therapist Program and Illinois PT license.
• Minimum of 1 year of physical therapy experience, ideally in home healthcare.
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Good clinical judgement, strong interpersonal skills, dexterity and compassion.

Responsabilités

• Provide home-based physical therapy to patients with mobility limitations in a home setting.
• Collaborate with RNs, LPNs, CNAs, HHAs, OTs and SLPs based on patient care plan.
• Observe movements, document progress and adjust care plans as needed.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients on plan of care, goals, exercises and self-care.
• Coach family on the plan of care and safe at-home exercises.

Responsibilities

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed
• Assist patient with exercises and hands-on therapy to manage pain and increase mobility
• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

Requirements
