Full-Time Physical Therapist - Home Health

Full-time

Our client is seeking a Physical Therapist in Houston, Texas! If you have experience as a PT and are looking to expand to the home health setting, this opportunity could be the perfect fit for you!

** This role is for the Houston, TX, area**

We are looking for a registered PT professional who provides coordinated care to patients of all age groups. Plans, implements, and evaluates patient care plans to restore or maintain patient well-being. Plans organizes and conducts physical therapy treatment programs based on the medical referral and their evaluation. Collaborates with interdisciplinary teams.

  • Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs and determine the level of functioning and appropriateness of patient for home care.
  • Develops and evaluates the care plan in partnership with the patient, representative (if any), and caregiver(s).
  • Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.
  • Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members, when applicable, to perform patient care that is consistent with patients’ needs.
  • Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
  • Accepts clinical assignments that are consistent with education and competence to care for patients. Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.
  • Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
  • Communicate with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
  • Meets mandatory continuing education requirements of the Agency and licensing board.
  • Assists and instructs patients and families in transfer techniques appropriate to patient abilities. Instructs on the use of equipment such as walkers, wheelchairs, canes, etc., in a safe manner.
  • Utilizes appropriate therapy equipment specific to patients’ diagnoses and needs.
  • Provides patient, caregiver, and family counseling; Provides patient and caregiver education.
  • Uses effective interpersonal relations and communication skills.
  • Licensure: Active, state license as a physical therapist or a multi-state license issued by a Physical Therapy Licensure Compact (PTLC) member state.
  • Experience: 1 year of experience as a physical therapist in an acute care, rehabilitation, or home health setting is preferred. The client is willing to train.

Skills: Therapy skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR certification. Knowledge of durable medical equipment.

EMR: Kinnser experience preferred but not required
