Job summary

Hemet Hills Post Acute in Hemet, CA, is seeking a dedicated Physical Therapist to join our team. You will assess patients, diagnose movement dysfunction, and develop personalized treatment plans to improve mobility and recovery.

Expect hands-on therapy, education for patients, and progress tracking in a post-acute setting. Applicants must be a graduate of an accredited Physical Therapy program with an active CA license.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• CA license active and in good standing is required.
• Experience in post-acute or long-term care setting preferred.

Responsibilities

• Consult with patients to assess condition and review medical history.
• Diagnose movement dysfunction by observing patient movements.
• Develop individualized treatment plans for recovery.
• Teach therapeutic exercises and stretches to improve condition.

Hemet Hills Post Acute is a newly acquired skilled nursing facility located in Hemet, CA! With 178 beds, servicing both short- and long-term patients; we are looking for the best nurses and staff to join our team. We look forward to meeting you soon!We offer competitive pay, great benefits, excellent culture with appreciation rewards, healthcare and 401k, training, and career advancement opportunities.Pay: From $55 - 58 per hour DOEDuties:• Consult with patients to learn about their physical condition and symptoms and review their medical history and referrals from doctors or surgeons• Diagnose movement dysfunction by listening to patients and observing them as they move about• Develop an individualized treatment plan• Coach patients and teach them therapeutic exercises and stretches to improve their condition• Use hands-on therapy such as massage to ease patients’ pain and provide stimulation to promote healing• Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly• Advise patients and their families on in-home treatment options and what to expect from the recovery processAdditional Requirements:Must be a graduate of an accredited Physical Therapy ProgramExperience in providing appropriate care in post-acute or long-term care environment (preferred)Current, unencumbered, active CA Physical Therapy License (required)CPR (as requested)COVID Vaccine Required or Valid Exemption.We are an equal opportunity employer.
