Hemet Hills Post Acute, a newly acquired skilled nursing facility in Hemet, CA, seeks a licensed Physical Therapist (PT) to assess patients, design individualized plans, and guide recovery through exercises and hands-on therapy.

Formación

• Graduate of an accredited Physical Therapy Program.
• Experience in post-acute or long-term care settings preferred.
• Current, active CA Physical Therapy License required.
• CPR as requested.
• COVID vaccine or valid exemption.

Responsabilidades

• Consult with patients to learn about their condition and review medical history.
• Diagnose movement dysfunction by observing patients.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic exercises.
• Use hands-on therapy to ease pain and promote healing.
• Maintain patient records and track goals/progress.
• Advise patients and families on in-home treatment expectations.
