Job summary

Life Care Centers of America is seeking a Physical Therapist to provide direct rehab care in physical therapy to patients referred by physicians, in a SNF setting. This role requires licensure and up-to-date CPR, with opportunities to advance into leadership through our career pathways.

You will work within an interdisciplinary team, manage treatment plans, and ensure patient-centered care while complying with regulatory standards and Life Care’s policies.

Qualifications

• Graduate of an accredited program in physical therapy (DPT/MSPT/BSPT)
• Current state licensure in good standing
• One year experience preferred; new grads welcome
• CPR certification upon hire or during orientation

Responsibilities

• Participates in community health matters/projects as appropriate
• Liaises with patients, families, support departments to plan patient needs
• Maintain confidentiality of all proprietary information
• Follow physical therapy treatment plans and modify goals as needed
• Oversee and evaluate care given by PTAs, Rehab Aides, and PT/PTA students
• Chart appropriately and timely
• Communicate effectively within an interdisciplinary team
