Macon Valley Nursing & Rehabilitation Center is seeking a dedicated Physical Therapist / PT to provide skilled physical therapy, assess and plan personalized interventions, and enhance residents’ functional independence in a 24/7 skilled nursing facility.

Responsibilities include supervising PTAs, collaborating with the care team, documenting care, and communicating with residents and families while upholding the highest standards of compassionate care.

Qualifications

• Valid physical therapy license
• Graduate of an accred­ited PT program
• Successful criminal background check
• Subject to pre-employment drug testing
• Long-term care facility experience preferred

• Collaborate with other health care professionals in assessment, planning, and safe implementation of therapy
• Be a resident advocate with the resident at the center of all we do
• Ensure excellent communication among residents, families, and staff
• Provide clinical documentation
• Create positive resident outcomes
• Treat others with kindness, compassion, honesty, and integrity
• Help create a culture of superior customer service

JOB SUMMARY

The Physical Therapist / PT position is to be responsible for providing skilled physical therapy. The physical therapist will assess, plan, and implement personalized physical therapy interventions to enhance residents' quality of life and functional independence.
