We are seeking a licensed and compassionate Physical Therapist (PT) to join our rehabilitation team and provide high-quality, skilled therapy services to residents in accordance with physician orders and individualized care plans.

Key Responsibilities
- Evaluate residents and develop individualized treatment plans based on clinical needs and physician orders.
- Provide skilled physical therapy services in accordance with established care plans.
- Monitor resident progress and modify treatment approaches as clinically appropriate.
- Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries.
- Collaborate with nursing, therapy, and interdisciplinary team members to achieve positive resident outcomes.
- Participate in care planning, utilization review meetings, and interdisciplinary conferences.
- Educate residents, families, and staff regarding therapy goals, treatment plans, safety, and functional mobility.
- Maintain established productivity, documentation, and quality standards.
- Support regulatory compliance and survey readiness.
- Promote resident safety, dignity, independence, and overall well-being.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Current and active state license as a Physical Therapist.
- Long-term care or skilled nursing experience preferred.
- Strong clinical documentation and communication skills.
- Excellent interpersonal and organizational skills.
- Ability to work effectively within an interdisciplinary team.
- Compassionate, patient-focused, and committed to delivering high-quality care.
Physical Requirements
- Ability to stand, walk, bend, and assist residents throughout the workday.
- Ability to safely lift, transfer, and support residents during therapy activities.
- Ability to perform physical tasks associated with patient care and therapeutic treatment.
- Comfortable working in a clinical and long-term care environment.
