Job summary

Life Care Centers of America seeks a Physical Therapist to provide direct rehab services in skilled nursing facilities. You will work with physicians to implement treatment plans for diverse patients, promoting functional progress in a patient-centered environment.

New grads are welcome, with opportunities for career advancement and leadership roles. A $15,000 sign-on bonus is offered along with a comprehensive benefits package for full-time staff.

Qualifications

• Graduate of an accredited physical therapy program (BSPT/MSPT/DPT).
• Currently registered/licensed in the applicable State.
• One year experience preferred; new grads welcome to apply.
• CPR certification upon hire or during orientation; CPR certification must remain current.

Responsibilities

• Participate in community health matters/projects as appropriate.
• Plan for patient needs with liaisons from patients, families, and departments.
• Follow physical therapy treatment plans for patients.
• Set, assess, and modify realistic, measurable goals.
• Oversee and evaluate care given by PTAs and rehab aides and students.
• Chart appropriately and timely.
• Utilize therapy software accurately.
• Work within an interdisciplinary team to ensure quality care.
