Qualifications

Responsibilities

• Evaluates assigned patients and collects data from records, interviews and observations.
• Develops and implements individualized treatment plans.
• Provides direct therapy using various modalities and coordinate with rehab, medical and nursing care plans.
• Documents evaluation results, treatment plans, progress notes and reports.
• Communicates patient status and needs with patient, family and multidisciplinary team.
