Job summary

Avera Therapy Center-Aberdeen is seeking a licensed Physical Therapist to coordinate and deliver therapy services at the Aberdeen site. You will evaluate patients, design individualized treatment plans, and apply evidence-based interventions to promote recovery and function.

You will document assessments, progress, and treatment plans while communicating with patients, families, and the care team. Ongoing education and interdisciplinary collaboration are encouraged.

Qualifications

• Active Physical Therapist license in the state of practice.
• Strong communication skills with patients and care teams.
• Ability to meet schedule hours and adapt to rotating weekends/holidays.

Responsibilities

• Evaluates assigned patients using records, interviews, observations and team input.
• Develops and implements individualized treatment programs.
• Delivers direct therapy across varied settings within patient care plans.
• Documents evaluation results, treatment plans and progress notes.
• Communicates patient status and needs with family, physicians and team.
• Collaborates with interdisciplinary team throughout the intervention program.
• Pursues ongoing professional development and current clinical literature.
