Job summary

Avera Gregory Hospital in South Dakota is seeking a Physical Therapist to coordinate and deliver therapy services, implementing individualized treatment programs for patients and patient groups.

Qualifications

• Active license to practice physical therapy in the state.
• Prior clinical experience preferred.

Responsibilities

• Evaluates assigned patients, using data from records, interviews and team input.
• Develops and implements individualized treatment plans for patients.
• Instructs patient and caregivers in home programs or exercises.
• Adapts intervention strategies based on ongoing evaluation.
• Provides direct therapy in various settings and coordinates with rehab and nursing care.
• Prepares documentation such as evaluation results and progress notes.
• Communicates with patient, family, physician, and team; attends care conferences.
• Oversees PT assistants and students to ensure safe patient care.
