Job summary

Avera St Luke’s Hospital is seeking a licensed Physical Therapist to join our multidisciplinary team. You will coordinate and deliver physical therapy services, implementing treatment programs for individual patients and groups in alignment with department objectives.

Responsibilities include evaluating patients, developing personalized plans, instructing patients and families, coordinating with rehab, medical and nursing staff, and documenting progress.

Qualifications

• Active Physical Therapist license in the state of practice.

Responsibilities

• Evaluates assigned patients and gathers data from records, interviews, and team members.
• Develops and implements individualized treatment programs for patients.
• Instructs patients and families in home programs or exercises.
• Provides direct therapy services in various settings and coordinates with rehab, medical, and nursing care plans.
• Maintains documentation and communicates patient status with the care team.
• Oversees PT assistants, techs and students to ensure safe patient care.
• Maintains knowledge of proper use of equipment for patient care.
