Job summary

Avera Queen of Peace Hospital is seeking a dedicated Physical Therapist to coordinate and deliver PT services, implementing individualized treatment plans for patients and groups in alignment with department goals.

You will evaluate patients, develop, modify, and document treatment plans, and communicate with families and the care team. A strong interdisciplinary approach and ongoing professional development are encouraged.

Qualifications

• Active license to practice physical therapy in the state of practice.

Responsibilities

• Evaluates assigned patients and gathers data from records, interviews, observation and team members.
• Develops and implements individualized treatment programs for patients needing services.
• Provides direct therapy services using various settings and plans coordinated with the rehabilitation, medical, and nursing care plans.
• Prepares documentation such as evaluation results and treatment plans.
• Communicates with patient, family, physician, and interdisciplinary team regarding progress and needs.
• Establishes rapport and collaborates with interdisciplinary team throughout the patient’s intervention program.
• Engages in ongoing education and professional development through courses and workshops.
