Job summary

Saber Health Group is hiring a Physical Therapist at Kempsville Health and Rehab in Virginia Beach, VA to help residents regain independence in a SNF setting. You will evaluate, design treatment plans, and guide patients toward functional goals with education and personalized exercise programs.

The role requires a bachelor’s degree in Physical Therapy and an active license; familiarity with EHR systems is preferred.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Active, unencumbered license to practice in the state of employment.
• Basic computer skills and ability to navigate electronic health records.

Responsibilities

• Return patients to optimal physical function in SNF setting.
• Instruct patients in maintaining function post-discharge with education and independent exercise.
• Develop and implement individualized therapy plans in collaboration with the care team.

Position Summary:

The primary function of your job is to return patient to optimal physical function and facilitate the achievement of patient, physician, and therapy goals within a SNF setting. Instruct the patient in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification.

Education/Qualifications:

• Employee must possess a bachelor’s degree or greater in Physical Therapy from an accredited college or university.

• Must maintain an active, unencumbered license granted from state of employment.

• Must possess basic computer skills and ability to navigate through various software programs, including service delivery software and the electronic health record software.

Shift & Wages:

• Full-Time and Part-Time Opportunities Available

• Shift: Varies

• Wage:

  • Full-Time: $39.00 - $50.00 per hour

  • Part-Time: $40.00 - $53.00 per hour

• Sign-on Bonus: $10,000 for Full-Time

• Refer a Friend Bonus: $5,000
