Job summary

Cherrydale Post Acute in Greenville, SC seeks a Physical Therapist to evaluate and treat residents, improving mobility and independence. You will work with physicians, families, and an interdisciplinary team to deliver high-quality, patient-centered care.

The role emphasizes creating individualized treatment plans, supervising PTAs and staff, and contributing to care planning and discharge planning. A SC PT license and SNF rehab experience are preferred.

Qualifications

• Bachelor’s degree in Physical Therapy required.
• SC Physical Therapist license required.
• Experience in skilled nursing or rehab setting preferred.
• Continued education and licensure in good standing required.

Responsibilities

• Perform patient evaluations within 24 hours (or 1 business day) of referral.
• Develop and implement individualized treatment plans.
• Provide therapy services to improve strength, balance, mobility, and function.
• Monitor patient progress and adjust treatment plans as needed.
• Maintain accurate and compliant clinical documentation.
• Participate in care planning, rehab conferences, and discharge planning.
• Educate residents, families, and staff on therapy programs and goals.
• Recommend and assist with durable medical equipment (DME) needs.
• Conduct in-service training and staff education.
• Supervise PTAs, aides, and students.

Position Summary

• Active, unencumbered Physical Therapist license in South Carolina (required)
• Must maintain all continuing education and remain in good standing with the State Board
• Experience in a skilled nursing or rehab setting preferred
• Familiarity with PointClickCare (PCC) and Casamba preferred
• Strong clinical, communication, and leadership skills
