Job summary

Cherrydale Post Acute in Greenville, SC is seeking a Physical Therapist (PT) for PRN work. You will evaluate residents, develop individualized treatment plans, and work with an interdisciplinary team to improve mobility and independence.

The role offers flexible scheduling and opportunities for career advancement within our rehab program. The PT must hold a Physical Therapy degree and SC license; nursing facility experience and PCC/Casamba familiarity are preferred.

Qualifications

• Physical Therapy degree required; South Carolina license preferred.
• Experience in skilled nursing or rehab setting preferred.
• Familiarity with PCC and Casamba preferred.

Responsibilities

• Perform patient evaluations and develop individualized treatment plans.
• Treat residents to improve mobility, strength, balance and independence.
• Monitor progress and adjust plans as needed.
• Maintain accurate clinical documentation.
• Communicate with physicians, residents, families and care team.
• Participate in care and discharge planning.
• Conduct staff education and in-service training.
• Supervise PTAs, aides and students as needed.
