Job summary

PACS in Santa Cruz is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team at Santa Cruz Post Acute. You will evaluate and treat residents to restore mobility and independence, working with an interdisciplinary team.

We offer flexible schedules including full-time, part-time, and PRN, with starting pay at $50-58 per hour DOE plus benefits. California licensure and a passion for patient-centered care are required.

Qualifications

  • Current Physical Therapist (PT) license in California.
  • Graduate of an accredited Physical Therapy program.
  • SNF experience preferred but not required-new graduates are welcome!
  • Strong communication, documentation, and time-management skills.

Responsibilities

  • Evaluate residents and create personalized treatment plans.
  • Provide gait training, strengthening, balance, pain management.
  • Document evaluations, progress notes, and discharge summaries.
  • Collaborate with nursing, therapy team, physicians, and families.
  • Educate residents and caregivers on exercise and safety.
  • Participate in interdisciplinary care conferences.
  • Ensure compliance with state and federal regulations.

Full-Time Part-Time PRN


Starting at $50-58/hour DOE + Benefits


Santa Cruz Post Acute is seeking a dedicated and compassionate Physical Therapist (PT) to join our skilled rehabilitation team. We deliver patient-centered care in a supportive, collaborative setting where your expertise truly makes a difference. Full-time, part-time, and PRN opportunities are available.


Position Summary

The Physical Therapist is responsible for evaluating, planning, and providing skilled physical therapy services to help residents restore strength, mobility, balance, and functional independence. This role works closely with the interdisciplinary care team to ensure high-quality outcomes.


Responsibilities


  • Conduct comprehensive evaluations and develop individualized treatment plans for residents with physical and functional impairments.

  • Provide therapeutic interventions including gait training, strengthening, balance activities, pain management, and mobility training.

  • Document evaluations, daily notes, progress reports, and discharge summaries in accordance with regulatory and facility standards.

  • Collaborate closely with nursing staff, therapy team, physicians, and families to support resident goals.

  • Educate residents and caregivers on exercise programs, safety techniques, and mobility strategies.

  • Participate in interdisciplinary care conferences and contribute to overall care planning.

  • Maintain compliance with state, federal, and professional practice regulations.


Qualifications
