Santa Cruz Post Acute is seeking a dedicated Physical Therapist (PT) to join our skilled rehabilitation team in Santa Cruz. We offer full-time, part-time, and PRN options.

The PT will evaluate residents, plan treatment, and provide therapies to improve strength, mobility, balance, and independence. Qualifications

• Current Physical Therapist license in California (CA).
• Graduate of an accredited Physical Therapy program.
• Strong communication, documentation, and time-management skills.
• SNF experience preferred but not required—new graduates welcome.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans for residents.
• Provide gait training, strengthening, balance activities, pain management, and mobility training.
• Document evaluations, daily notes, progress reports, and discharge summaries.
• Collaborate with nursing, therapy team, physicians, and families to support resident goals.
• Educate residents and caregivers on exercise programs, safety techniques, and mobility strategies.
• Participate in interdisciplinary care conferences and contribute to care planning.
• Maintain compliance with state, federal, and professional practice regulations.
