Santa Cruz Post Acute is seeking a dedicated Physical Therapist (PT) to join our skilled rehabilitation team in Santa Cruz. You will evaluate, plan, and provide skilled PT services to help residents improve strength, mobility, balance, and independence.

You will collaborate with the interdisciplinary care team to ensure high-quality outcomes. We offer full-time, part-time, and PRN opportunities with starting pay at $50 per hour (DOE) plus a comprehensive benefits package.

Qualifications

• Current Physical Therapist (PT) license in California.
• Graduate of an accredited Physical Therapy program.
• Strong communication, documentation, and time-management skills.
• SNF experience preferred but not required—new graduates are welcome!

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans.
• Provide gait training, strengthening, balance activities, pain management, and mobility training.
• Document evaluations, daily notes, progress reports, and discharge summaries.
• Collaborate with nursing staff, therapy team, physicians, and families.
• Educate residents and caregivers on exercise programs and safety techniques.
• Participate in interdisciplinary care conferences and contribute to overall care planning.
• Maintain compliance with state, federal, and professional practice regulations.
