Job summary

Santa Rosa Post Acute is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team in Santa Rosa, CA. You will evaluate residents, develop individualized treatment plans, and provide evidence-based PT services to improve mobility, balance, and independence in a skilled nursing/post-acute setting.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current California Physical Therapist license.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively within an interdisciplinary healthcare team.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide evidence-based physical therapy services to improve mobility, strength, balance, and functional independence.
• Assess patient needs and establish measurable treatment goals.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing staff, therapy team members, and families.
• Educate patients and caregivers regarding treatment plans, exercises, and safety recommendations.
• Participate in interdisciplinary care planning meetings and patient conferences.
• Supervise Physical Therapist Assistants (PTAs) as appropriate.
• Recommend adaptive equipment and mobility aids when necessary.
• Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines.

• 50% after 45 days of continuous employment
• 50% after 90 days of continuous employment
• Employee must remain actively employed and in good standing at the time of payout
