Position Overview

• Rate: $65 (DOE)
• Experience: Experienced PTs or New Grads Welcome!
• Location: San Jose, CA
• Role: Evaluate and treat residents, lead clinical training, and manage individualized rehabilitation plans.
• Clinical Excellence: Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders.
• Treatment Planning: Develop and implement individualized plans focused on improving mobility, strength, balance, and functional independence.
• Supervision & Training: Supervise Physical Therapy Assistants (PTAs) and assist the nursing department with the training of Restorative Aides.
• Collaboration: Communicate regularly with physicians, nursing staff, and families regarding progress, goals, and discharge planning.
• Documentation: Complete timely and accurate clinical documentation in compliance with facility, state, and federal requirements.
• Equipment Management: Recommend and facilitate the ordering of durable medical equipment (DME) such as walkers, canes, and splints to enhance resident independence.
• Education: Provide in-service training for facility staff and educate families on fall prevention and home exercise programs.

Qualifications

• Education: Graduate of an accredited Physical Therapy program.
• Licensure: Current and active California Physical Therapist license (or eligibility for new grads).
• Skills: Ability to relate positively and effectively with residents, families, and professional colleagues.
