Job summary

White Blossom Care Center in San Jose, CA is seeking a motivated Physical Therapist Assistant (PTA) to join an elite in-house rehabilitation team. The center features the Bay Area’s largest rehab gym and on-site modalities, with flexible full-time scheduling and a supportive culture.

You'll provide patient-centered therapy, document progress, participate in interdisciplinary care, mentor Restorative Aides, and educate residents and families on ongoing care and safe discharge planning.

Qualifications

• Active California Physical Therapist Assistant license is required.
• Commitment to patient-centered care and ongoing professional growth.
• Experience in a fast-paced clinical setting is preferred.

Responsibilities

• Deliver patient treatment as directed by the PT per physician orders.
• Complete daily treatment notes and weekly progress per PT standards.
• Participate in interdisciplinary care planning and team conferences.
• Assist in training Restorative Aides with the nursing team to promote mobility.
• Educate residents, families, and caregivers on maintenance and discharge planning.
• Maintain rehab equipment in excellent working condition.
