Job summary

White Blossom Care Center in San Jose, CA is seeking a motivated Physical Therapist (PT) to join our rehabilitation team. We welcome new grads and experienced clinicians to join a supportive skilled nursing environment with a comprehensive benefits package.

The PT will evaluate and treat residents, lead clinical training, supervise PTAs, and manage individualized rehabilitation plans. Join a team focused on resident recovery and independence in a collaborative setting.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and active California Physical Therapist license (or eligibility for new grads).
• Strong interpersonal and communication skills with residents, families and staff.
• Commitment to resident rights and confidentiality.

Responsibilities

• Evaluate residents to determine PT needs, functional limitations and rehabilitation potential.
• Develop and implement individualized treatment plans to improve mobility, strength and independence.
• Supervise Physical Therapy Assistants (PTAs) and assist with training Restorative Aides.
• Collaborate with physicians, nursing staff and families on progress, goals and discharge planning.
• Complete timely and accurate clinical documentation in compliance with regulations.
• Recommend and facilitate ordering of durable medical equipment (DME) to support independence.
• Provide in-service training for staff and educate families on fall prevention and home exercise programs.
