Job summary

Central Gardens Post Acute in San Francisco's Fillmore District is seeking a compassionate Physical Therapist to join our in-house rehabilitation team. We offer Full-Time, Part-Time and PRN opportunities with flexible scheduling to support your professional and personal needs.

You will perform evaluations, devise individualized plans, and deliver skilled therapy to residents, focusing on strength, mobility, balance and independence.

Qualifications

• Current California Physical Therapist license in good standing.
• Strong communication and interpersonal skills.
• Ability to work in an interdisciplinary team.
• New graduates encouraged to apply.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
• Provide skilled physical therapy services based on residents’ needs and goals.
• Implement evidence-based interventions to improve strength, mobility, balance, endurance, and functional independence.
• Monitor resident progress and adjust treatment plans as appropriate.
• Educate residents and families regarding therapy goals, exercises, mobility, and safety.
• Collaborate with PTAs and the interdisciplinary care team to support positive clinical outcomes.
• Maintain accurate and timely clinical documentation.
• Promote resident safety, dignity, independence, and quality of life.
• Participate in care planning and interdisciplinary team activities as needed.
