Central Gardens Post Acute in San Francisco is seeking a licensed Physical Therapist to join our in-house rehabilitation team. We offer a supportive environment for new graduates and seasoned clinicians alike, with flexible scheduling and mentorship to grow your clinical practice.

Responsibilities

• Evaluate residents and develop individualized, evidence-based treatment plans.
• Provide skilled physical therapy services for short-term and long-term care residents.
• Restore mobility, improve strength and balance, reduce fall risk, and maximize independence.
• Collaborate with physicians and the interdisciplinary care team to achieve resident outcomes.
• Supervise and mentor Physical Therapist Assistants as appropriate.
• Educate residents, families, and staff on mobility, safety, and restorative care techniques.
• Maintain accurate documentation in accordance with regulatory and facility standards.
