Job summary

Edenbrook Pine Haven, a leading skilled nursing facility, seeks a PRN Physical Therapist to deliver evidence-based rehab care. You will develop individualized therapy plans and guide patients toward independence in a family-like environment.

Requirements include a DPT or MPT and an active Minnesota PT license; SNF/home health experience is preferred. On-site role with competitive wages and supportive leadership.

Qualifications

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy (MPT) from an accredited program.
• Active Minnesota PT license in good standing.
• Experience in skilled nursing, home health, or rehabilitation settings preferred.

Responsibilities

• Develop and implement individualized therapy programs to restore and enhance patient performance.
• Refer patients to other disciplines as needed to address comprehensive goals.
• Instruct and train patients and caregivers to promote independence and QoL.
• Supervise PTAs, provisional licensees, students, and staff per licensure standards.
• Develop personalized care plans using various treatment techniques.
• Create fitness and wellness programs tailored to patient needs.
• Provide quality, evidence-based care with proven interventions.
• Motivate patients to reach rehabilitation goals.
• Promote healthy lifestyles by improving strength, balance, and coordination.
• Continuously evaluate progress and communicate with the team.

Position PRN Physical Therapist (PT)

Primary Job Responsibilities:

• Develop and implement individualized therapy programs to restore, reinforce, and enhance patient performance.
• Identify the need for and make referrals to other healthcare disciplines as necessary to address the comprehensive needs of the patient.
• Instruct, educate, and train patients and caregivers in skills and functions essential for promoting patient independence, improving quality of life, and diminishing or correcting pathology.
• Supervise Physical Therapist Assistants, provisional licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
• Develop personalized care plans using a variety of treatment techniques.
• Create fitness and wellness programs tailored to the specific needs of patients.
• Provide quality, personalized, and evidence-based care using proven interventions.
• Motivate patients during treatment to help them function optimally and reach their rehabilitation goals.
• Promote clients’ healthy lifestyles by improving their strength, flexibility, balance, and coordination.
• Continuously evaluate patient progress, monitor outcomes, and communicate updates effectively with the interdisciplinary team.
