Job summary

Grandview Nursing & Rehabilitation Center in Danville, PA is seeking a compassionate Part-Time Physical Therapist (PT) to join our rehabilitation team in a SNF/LTC setting. The role offers opportunities for growth and hands-on experience while collaborating with an interdisciplinary team, nursing staff, physicians, and therapists.

The ideal candidate will be a graduate of an accredited PT program, hold PA PT licensure or be eligible, and demonstrate strong communication and patient-centered

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapist (PT) license or eligibility for licensure.
• Previous SNF/LTC experience preferred but not required.
• Strong communication, organizational, and interpersonal skills.
• Compassionate, patient-centered approach to care.
• Ability to work independently while contributing to a collaborative rehabilitation team.
• Familiarity with Net Health and PCC is a plus, but not required.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy to improve mobility, strength, balance and independence.
• Assist residents to achieve their highest possible level of independence and quality of life.
• Collaborate with nursing staff, physicians, and other therapists to coordinate care.
• Maintain accurate and timely clinical documentation using Net Health.
• Participate in interdisciplinary care planning and progress discussions.
• Educate residents, families, and caregivers on exercises, mobility techniques and safety.
• Ensure compliance with state, federal and facility regulations.

Responsibilities
