Job summary

Kadima Health and Nursing at Pottstown is seeking a licensed Physical Therapist to join our team in Pottstown, PA. You will evaluate residents, develop treatment plans, and provide hands-on therapy to improve function and quality of life.

Experience in SNF/long-term care and proficiency with Net Health documentation are preferred. You will collaborate with care teams, document progress, and ensure compliance with state and federal regulations.

Qualifications

• Experience in SNF/long-term care preferred.
• Current knowledge of physical therapy treatment practices.
• Familiarity with Net Health documentation system preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Evaluate, plan, and administer treatment to residents and patients to relieve pain and improve function.
• Assure all treatment is delivered in accordance with an established plan of care.
• Provide clinical support and instruct patients, families, and caregivers.
• Measure and record patient’s motor function, strength, and muscle performance.
• Monitor patient responses to treatments and modify as needed.
• Complete documentation in accordance with company policy and state/federal regulations.

The Physical Therapist is expected to follow facility core values in administering department services and helping to fulfill all therapy-related resident care needs.

Duties/Responsibilities:

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.

• Assured all treatment is delivered in accordance with an established plan of care

• Provide clinical support and instruct patients, families, and caregivers

• Measures & records patient’s motor function, strength, and muscle performance

• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary

• Completes documentation in accordance with company policy and state/federal regulations

Skills:

• SNF/Long term care experience preferred

• Current knowledge of treatment practices

• Knowledge of Net Health Electronic Documentation System preferred

• Ability to manage patients with different types of personalities

Required licensure:

• Current License as a Physical Therapist in the State of PA. Must be in good standing.
