Job summary

Kadima Health and Nursing at Palmyra is seeking a licensed Physical Therapist to join our care team in Palmyra, PA. You will evaluate residents, plan and administer treatments, and support families and caregivers in accordance with a plan of care.

Responsibilities include documenting progress, measuring motor function, and adjusting treatments as needed while upholding facility values and regulatory compliance. SNF/long-term care experience is preferred, PA licensure in good standing required.

Qualifications

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsibilities

• Assured all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Measures & records patient’s motor function, strength, and muscle performance
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
• Completes documentation in accordance with company policy and state/federal regulations
