Job summary

Kadima Health and Nursing at Palmyra is seeking a Physical Therapist to join our team. The therapist will evaluate residents, plan and administer treatments to relieve pain and improve function in SNF/long-term care settings.

The role requires current PA licensure, knowledge of up-to-date treatment practices, documentation in Net Health EDS, and the ability to work with diverse patients. Experience in SNF/long-term care preferred.

Qualifications

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsibilities

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Measures & records patient’s motor function, strength, and muscle performance
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
• Completes documentation in accordance with company policy and state/federal regulations
