Job summary

Kadima Health and Nursing at Luzerne in Pennsylvania is seeking a dedicated Physical Therapist to join our care team. The role focuses on evaluating, planning, and administering therapies to residents in SNF/long-term care settings, with emphasis on restoring function and promoting healing.

You will document progress, adjust treatment plans as needed, and educate patients, families, and caregivers. A PA license in good standing is required, and SNF/long-term care experience is preferred.

Qualifications

• Current License as a Physical Therapist in the State of PA. Must be in good standing.

Responsibilities

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Measures & records patient's motor function, strength, and muscle performance
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
• Completes documentation in accordance with company policy and state/federal regulations

The Physical Therapist is expected to follow facility core values in administering department services and helping to fulfill all therapy-related resident care needs.

Duties/Responsibilities:

Skills:

• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different types of personalities

Required licensure:

• Current License as a Physical Therapist in the State of PA. Must be in good standing.
