Kadima Health and Nursing at Lititz is seeking a Physical Therapist to join our team. The PT will assess residents, develop treatment plans, and deliver therapy aimed at reducing pain and improving function in a skilled nursing setting.

Responsibilities include following established care plans, documenting progress, educating residents and families, and adjusting treatment as needed while meeting state and company policies.

Qualifikationen

• Experience in SNF/long-term care settings is preferred.
• Knowledge of current treatment practices for physical therapy.
• Familiarity with Net Health Electronic Documentation System.
• Ability to work with residents with diverse personalities.

The Physical Therapist is expected to follow facility core values in administering department services and helping to fulfill all therapy-related resident care needs.

Duties/Responsibilities:

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.

• Assured all treatment is delivered in accordance with an established plan of care

• Provide clinical support and instruct patients, families, and caregivers

• Measures & records patient’s motor function, strength, and muscle performance

• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary

• Completes documentation in accordance with company policy and state/federal regulations

Skills:

• SNF/Long term care experience preferred

• Current knowledge of treatment practices

• Knowledge of Net Health Electronic Documentation System preferred

• Ability to manage patients with different types of personalities

Required licensure:

• Current License as a Physical Therapist in the State of PA. Must be in good standing.
