Job summary

Spring Hill Rehabilitation & Nursing Center is seeking a dedicated Physical Therapist (PT) to join our team. Design and implement individualized therapy plans to help patients regain function and independence, collaborating with a multidisciplinary care team.

Responsibilities

• Conduct thorough initial and ongoing patient assessments to identify physical therapy needs and goals
• Develop and implement individualized physical therapy plans in collaboration with our patients, families, and healthcare team
• Provide physical therapy interventions to patients, including exercises, education, and manual therapy techniques
• Monitor and document patient progress, adjusting physical therapy plans as necessary
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
• Participate in patient care conferences and treatment team meetings to discuss patient progress and care plans
• Maintain accurate and timely patient records and reports
• Collaborate with other therapy disciplines (occupational therapy and speech therapy) to ensure comprehensive patient care
